Do I need to translate my vaccination card for a US school?

Yes, if it is in Portuguese. Enrollment offices and school nurses have to check your child's doses against the state's immunization schedule, and they can only check what they can read. A certified translation, dose by dose, lets the nurse tick each requirement without calling you back for a version she can trust. Kindergarten, public school, university registration, and even summer camps run on the same checklists.

What does USCIS require for vaccination records?

USCIS never sees your caderneta directly. What it receives is Form I-693, completed and sealed by a civil surgeon, and the surgeon is the one who reads your vaccination records to fill in the vaccine table. The Brazilian Pentavalente does not exist under that name on the US schedule, and BCG is not on the US schedule at all. Lost the card? Your records probably still exist

Brazilians lose the paper caderneta all the time, and the doses usually survive it. Vaccines taken at public health posts are registered in the national system, so the Meu SUS Digital app can show doses you thought were gone, and the UBS or clinic where you were vaccinated keeps its own books you can request a copy from.
